Important Note:
If you are experiencing any of the following symptoms, it is considered an emergency, requiring care at a local emergency department:
- chest pain or pressure
- coughing up or vomiting blood
- severe abdominal pain
- difficulty speaking
- fainting or weakness
- severe or persistent vomiting or diarrhea
- shortness of breath
- sudden change or loss of vision
- sudden or severe pain
- suicidal feelings
- uncontrolled bleeding
To become a new patient:
Complete the new patient registration form and the new patient history form. Please note that these forms are for patients who are not currently registered with HopeHealth.

You can return these forms to HopeHealth by e-mail, taking them to a HopeHealth front desk, or by mailing them to the appropriate HopeHealth location. Printed forms are also available at each of our locations. Please note which location you are requesting services.
Please bring to your first appointment:
- All medications in original containers (including over-the-counters, natural and herbal medicines, and vitamins)
- Insurance card or other insurance information
- All medical records from previous providers
- Any form of identification
- Proof of income and household size (if interested in the Sliding Fee Discount Program)
