The HopeHealth Medical Plaza in Florence opened its doors in February 2016 followed by a new Phase II wing which added approximately 40,000 square feet to the facility in 2017.
Now, we are expanding the footprint. The Streater building at the corner of Dargan and Darlington Street was purchased by HopeHealth in 2018. The structure is currently undergoing renovation with an estimated completion of 2025. The location will provide value-based community services outside the traditional health care scope. Services will focus on seniors, Veterans, and Black men in the community. This project aims to help strengthen community engagement and decrease barriers to care by enhancing the availability and accessibility of health programs.
The campus renovation and expansion will also include a retention pond and walking trail, enhancing and promoting wellness and community.

As a patient-centered medical home, HopeHealth’s medical team works to prevent and detect illness and the early onset of disease, provide routine physical examinations, and promote overall healthy lifestyles. Our health care providers offer access to basic health care services and integrate a range of health resources to provide the very best in patient care, prevention, and support services.

Suite A is the first set of medical offices located on the first floor of the Medical Plaza. Access Hope, our extended hours health care service, is located in this suite. Health care providers in this area specialize in family medicine. Hours are Saturday from 8 am – 8 pm.
Suite B is the second set of medical offices located on the first floor of the Medical Plaza. Health care providers in this area specialize in family medicine.
Suite C is the third set of medical offices located on the first floor in Phase 1. Health care providers in this area specialize in family medicine.

Suite E is the first set of medical offices located on the second floor in Phase 1. Health care providers in this area specialize in internal medicine and family medicine.

Suite I is the first set of medical offices located on the first floor in Phase 2. The Substance Use Disorder program and psychiatry is located in this suite.

Suite K is the third set of medical offices located on the first floor in Phase 2. Health care providers in this area specialize in Infectious Disease.
Suite L is the first set of medical offices located on the second floor in Phase 2. Providers in this suite focus on young adults, women’s health, and annual wellness visits.
Suite M is the second set of medical offices located on the second floor in Phase 2. Health care providers in this area specialize in Dentistry.
Suite N is the third set of medical offices located on the second floor in Phase 2. Health care providers in this area specialize in Endocrinology, Diabetes, and Nutrition.
Suite O is the fourth set of medical offices located on the second floor in Phase 2. Health care providers in this suite specialize in endocrinology, diabetes, and annual wellness visits.
Suite P is on the second floor in Phase 2 and offers annual wellness services.
The Diabetes Institute, located on the first floor, offers a variety of education, screening and counseling services related to diabetes and nutrition. Three endocrinologists, a nurse practitioner, diabetes educators, and registered dietician nutritionists provide evidence-based, proven methods for improving your health and wellness through nutrition, exercise, and lifestyle behaviors.
At HopeHealth, we go the extra mile to treat the whole person. Instead of focusing on a specific symptom, our providers and staff assist patients with determining the underlying causes of poor health. Sometimes this is as simple as providing coordinating transportation to appointments or assisting in making referrals to other social service providers. Other times, it’s a quick referral to a HopeHealth specialty service provider. Our integrated approach to health care truly places the patient at the center of care.
Primary Care Services
Conveniently located on the first floor, the HopeHealth Pharmacy offers over-the-counter and prescription medicine. Patients can pick up medications at one of three pick-up windows inside or use the drive-through window located at the rear of the building.
HopeHealth has a team of certified health insurance marketplace counselors that are trained to assist individuals in the Affordable Care Act enrollment process.
Our patient benefit counselors are available year-round to assist patients with a variety of financial services offered at HopeHealth. Counselors are available on the first floor, behind the reception area.
HopeHealth’s Language Services is a collective of staff tasked with providing support and guidance for patients who may not speak English well or in some cases not at all, and may need assistance navigating their doctors’ visits and other essential health care services.
Language Services
HopeHealth partners with LabCorp to offer general and specialty laboratory services to patients of all ages. Services range from routine tests, including basic blood count and cholesterol testing, to specialty lab tests. Lab services are located on the first floor and are available Monday through Friday, 8:00 am – 5:30 pm.
As our population ages, proactively addressing the health concerns of our senior patients can help provide greater quality of life and help seniors remain independent. HopeHealth provides primary health care services to seniors age 65 and older. Our providers help patients focus on their physical health, their mental health, and helps them manage multiple chronic conditions.
HopeHealth provides women’s health services across Florence, Williamsburg, and Clarendon Counties. Our board-certified gynecologist and nurse practitioners offer women’s health services to women of all ages. Our services are relevant to the lifespan of women, from routine gynecological exams and contraception method options to medication therapy for menopause. We also partner with evidence-based programs, non-profit organizations, and businesses to provide the best in women’s health services.
