HopeHealth provides chronic disease management for conditions such as hypertension, diabetes, heart disease, obesity, and asthma through various programs and services. We partner with nationally-recognized, evidence-based programs such as WISEWOMAN, Best Chance Network, and Better Choices Better Health. Your primary care provider will help customize your health care plan based on your individual needs. In some cases, patients will be assigned to a clinical case manager to help manage disease and care coordination.
Chronic Disease Management & Prevention Services
Arthritis – Your primary care provider will assess your arthritic condition and make recommendations that may include treatment such as pain management, chiropractic care, and massage therapy. If necessary, your provider will refer you to an orthopedic specialist for additional care.
Asthma – Your primary care provider will assist you in preventing untreated asthma, or diagnose and treat your existing condition. They will ensure that your medication regimen is current and effective. HopeHealth has referral arrangements with local Ear, Nose & Throat and Pulmonology providers for advanced care needs.
Cancer – Through routine examinations, your primary care provider will recommend routine screenings to help detect and prevent various types of cancer. Your provider will make referrals and coordinate care with oncologists and additional specialists on a needed basis. HopeHealth also provides annual breast and cervical cancer screenings for women through a partnership with the Best Chance Network.
Diabetes – HopeHealth offers blood glucose screenings to detect diabetes indicators within the clinical setting, as well as outreach events. Located at the HopeHealth Medical Plaza, our dieticians and nurse practitioner at the Diabetes Institute offer in-depth nutrition counseling and diabetes management in individual and group settings. Retinal eye exams for diabetes patients are available at the HopeHealth Medical Plaza – Diabetes and Nutrition Center. Additional, free educational opportunities are available to patients through the WISEWOMAN and Better Choices Better Health.
Heart Disease – Your primary care provider will help manage co-existing health conditions such as hypertension and diabetes and assess your risk of heart disease through routine screenings. Patients will receive a referral to a cardiologist or other specialist where needed.
Hypertension – HopeHealth provides screenings, education, and disease management of high blood pressure in the clinical setting and at health fairs in the community. Additional free educational programs, such as Better Choices Better Health and WISEWOMAN, are available to patients with hypertension and risk of stroke. Classes include: lifestyle choices, stress management, medication maintenance, blood pressure screenings, and more.
Obesity – Your primary care provider will provide routine screenings to diagnose as well as prevent obesity and assess the need for additional nutritional education counseling through the Diabetes and Nutrition Center. HopeHealth employees also provide BMI screenings at health fairs in the local community.
