Getting to Zero

HopeHealth remains committed to zero new HIV transmission by providing excellent medical care, access to life-saving medications, a range of support services, and effective prevention and education tools.

HIV Services

Though we now seek to provide affordable, accessible care to all, our mission began with the narrow but important goal of providing support, guidance and compassion to those affected by HIV/AIDS. Founded as a support group in 1991, we continue to offer medical care, medical case management, prevention and outreach services to those infected, affected and at-risk for HIV/AIDS.

HopeHealth offers three locations and a primary care site that provide comprehensive HIV treatment, prevention, and education services. The sites are known as Infectious Diseases Treatment and Prevention Centers. The Centers, in addition to HopeHealth Family Practice Kingstree, offer care and supportive services for people living with HIV in Florence, Darlington, Chesterfield, Marlboro, Marion, Dillon, Williamsburg, Clarendon, Orangeburg, Bamberg, Calhoun, Aiken, Barnwell, and Allendale counties.

Like many other service areas at HopeHealth, the service delivery model of the Infectious Diseases Treatment and Prevention Centers is based on an integration of care model encompassing three areas: clinic, community and client services.

Clinic Services consist of medical care and treatment related specifically to a patient’s diagnosis. The clinical staff team includes a medical office assistant, a nurse and a provider. The team meets with patients at least quarterly to assign and review patients’ medications, labs and coordinate any referrals for specialty care services.

Client Services include the coordination of patients’ medical and support services to: ensure clients receive medication, have access to support services (e.g. transportation, support groups, housing assistance, etc) and they remain engaged in care.

HopeHealth offers a range of supportive services for its infectious diseases patients in coordination with their medical treatment, including medical case management. HopeHealth employs a team of medical case managers to serve as coordinators and work directly with the clinical team to manage patients’ medical needs, medication regimens, referrals, other social services.

Community Services include partnering with local community organizations and agencies to provide free HIV and Hepatitis C screenings, as well as provide HIV, STD, HCV education and awareness, including evidence-based interventions.
