HopeHealth Infectious Disease Treatment & Prevention Centers offer a range of services including clinical care, case management, medication assistance, and community awareness and education.

Infectious Diseases

Infectious diseases are conditions caused by fungi, bacteria, parasites and/or viruses. Some infectious diseases can be passed from person to person. Some are transmitted by bites from insects or animals. And others are acquired by ingesting contaminated food or water or being exposed to organisms in the environment. There are multiple ways infectious diseases can be transmitted, such as contaminated food and/or water, or from person to person. Insect bites or wounds from animals, or other environmental exposures can also transmit infectious diseases.

Symptoms can vary based on the fungi, bacteria, parasites and/or virus causing the infection. Treatment for infectious disease can be broad, and depends on the infection itself. And, in many cases, infectious diseases can be prevented with vaccines and behavioral interventions.

The Infectious Disease Department at HopeHealth is committed to providing the highest quality of care and treatment to patients diagnosed with a range of acute and chronic infectious diseases. Our care model includes 5 board-certified physicians, nursing staff, and medical case managers offering specialized treatment of infectious diseases, as well as primary care and support services within one medical facility.

Like many other service areas at HopeHealth, the Infectious Diseases department is based on an integration of care model encompassing three areas of service: clinic, community, and client services.

Clinic Services include providing medical care for Infectious Diseases patients, ensuring clients receive medications and medication education, providing and/or coordinating referrals for additional services such as primary care, behavior health and substance use.

Client Services include the coordination of Infectious Diseases patients’ medical and support services to: ensure clients receive medication, have access to support services (e.g. transportation, support groups, housing assistance, etc), and they remain engaged in care.

Community Services include partnering with local community organizations and agencies to provide free rapid HIV & Hepatitis C screenings, as well as provide HIV, STD, HCV education and awareness, including evidence-based interventions. Additional services include locating, and helping new and former clients re-engage in care.
