Health centers are established in geographic areas where unmet health care needs have been identified. They exist to provide health care homes, to minimize unnecessary emergency department visits, and to catch the early onset of preventable chronic conditions. With the ongoing battle against chronic diseases in South Carolina, HopeHealth focuses on providing preventive services and programs for the communities we serve. In celebration of National Health Center Week, you can sign up on Wednesday, August 7 at any of our HopeHealth locations (from 9:00 am – 5:00 pm) to be a community health center advocate. We will also be signing people up to become registered voters.
